Here for you. Here for the community.

Since 1889, Flagler Health+ has been committed to caring for our friends, families and neighbors in St. Johns County. Our community has counted on us to be there during happy times like childbirth and survivorship, and through some of life’s most frightening health emergencies, illnesses and even a global pandemic. And we will continue to advance the physical, social and economic health of the community for generations to come.

Care Connect+, a social health program started by Flagler Health+, is transforming the way communities address social determinants of health. By connecting people with vital resources like stable housing, food pantries, mental health services and transportation, our care navigators are breaking down barriers and ensuring that all people have the opportunity to live their best life.

Whether you’re looking for a Primary Care physician for everyday wellness or care when you’re not feeling well, Flagler Health+ offers a variety of care options that fit your schedule and need.

Survived COVID-19

Desmond was admitted to Flagler Hospital with COVID-19 and was a medical miracle. Desmond’s two-month admission included a variety of precautions due to the contagious nature of the virus, including isolation from his young children. His ultimate goal was to see them in person again and, after the quarantine period ended and he was strong enough to go outside, his nurse was able to make his dream a reality. Our care teams know that caring for the whole patient – mind, body and spirit – involves more than medicine.
